What the contact form collects
Only what you type into it: your name, practice name (optional), phone number, email address, the service you are interested in and your message. Please do not include any patient information.

Where your enquiry goes
When you send the contact form, your details are delivered to us by email through FormSubmit (formsubmit.co).
Our WhatsApp buttons open WhatsApp with a message ready for you to send. Anything you send there is governed by WhatsApp’s own terms and privacy policy.
Your rights
The Protection of Personal Information Act (POPIA) gives you the right to ask what personal information is held about you, and to ask for it to be corrected or deleted, or to object to its use. Email mark@practiceinabox.co.za.
